Improve Confirmation Dialog ...
Some basic modifications to the Confirmation Dialog:
* Always attached to <body> regardless of view that called it.
* Always centered horizontally on screen.
* A black semi-transparent overlay is now displayed over everything, and under
the dialog.
* Various other style changes.
