Connor Peet 6b42d27b79 chat: polish agent session hover widget (#289750) 
Improves the UI and behavior of agent session hover widgets:

- Use provider icons instead of provider labels in hover headers for
  better visual consistency with the main sessions list
- Reorganize header details row to show provider icon • duration/time •
  diff stats • status (only if not completed). This makes the layout
  more compact and consistent with the list view
- Only show status badge in completed sessions to reduce visual clutter
- Fix hover widget layout refresh when displayed again without
  re-rendering to improve performance
- Limit hover rendering to in-progress or unread sessions to avoid
  expensive rendering for completed read sessions
